Good stuff.
I GM "on-the-fly". I set up the situation and give players options. If they do something dfferent than options given, I go with that. I have nothing in writting nor do I know what's going to happen next until player's tell me what their actions are and/or roll of the dice then I react accordingly. When next session rolls around I pick up where we left off and what we did; all from memory. It's kind of like all those sports guys who remember every little detail about stats and games. And I have 3-4 different games going at times. All on-the-fly.
Then my wife gets mad at me because I forgot to pick up milk and bread on the way home. (Probably rushing home to game).

I readily admit to running plot trains as the last group I ran IRL was made up of people with little gameing experiance and none of it playing Rifts. As they got more comfortable with the setting I started running a Villan's INC game as the players had ended up stopping in a little town for repairs and their were a couple groups of Badguys in the area. I ran the game that way because I knew that the group could not handle a Free form Matrix. As far as the Free Form Matrix goes it works best with smaller groups of players.
As far as Metagaming Personal Vendetta's go I have to say that they get offended when you call them on it. Back in '02 I gamed with a couple who did that very thing. The only reason they didn't go totally munchkin was that the President of the local College Role Playing Club could get all the books for the game that was being run and knew of things that made M & E's munchkin characters look like angry toddlers.

This is exactly how I GM as well.
