Give them a bonus to save vs. pain to start. I'd have the automatic save for a few more bio-e, maybe 10 or 15. Maybe let them function down to like -20 HP.

mole rats are technically endothermic in biology, but with such low metabolisms they are functionally exothermic. plus they have the ability to lower their already low metabolism even more in times of food shortage.
so the biological changes for a mutant molerat is less extreme than for say, a lizard. molerats already have all the heart configurations and such in place. a lizard needs to develop a 4 chamber heart, for example, to support the kind of metabolism the complex brain of an intellegent being needs.
mole rats just need to boost the metabolic rate.
in regards to respitory systems, i agree, with a few adjustments. birds and crocodilians would likely keep the airsac breathing system they had in unmutated form. it's way more efficient.
