Called Shot
I remember a rule somewhere that a "Called Shot" must be a roll of 12 or greater. Now is that a natural 12 or with bonuses?

With bonuses, but sometimes there are additional penalties for small targets. These are usually listed in the MDC by location section of the armor/robot. If not, you figure -3 for heads or -2 for limbs.. that sort of thing.
Also, called shots take 2 attacks and they go off on the second attack. So it's only usually ideal if you're off to the side during combat or the opponent is focused on another foe.
If you miss the called shot but still hit 8-11 then you hit but it goes to the main body instead.

the RUE states nothing like that. it is a 4 or higher for close combat and an 8 for ranged with bonuses a natural one always misses. The called shot is costly enough 2 attacks and doesn't get the +2 that a called aimed gets but costs 3 attacks. on top of it the called shot gets negatives for hitting a small target, and those negatives go up if the target is moving and moving evasively. so a level one character with the +1 to strike could have a -4 minimum to hit a head not moving. so they would have to roll an 11

How about in Fantasy setting. Example: Player A) wants to cut at player B) throat.
I consider this a called shot. So I should treat this as a -4 to strike Counts as 2 attacks but with bonuses, right?

no as the above is for ranged attacks, So if Player A is using a bow then yes, But a knife I would say yes to the penalty as it would be a small target and possibly covered by a gorget (i.e Neck armor).

To the OP its always with bonues as otherwise a trained military sniper would have the same chance to do a called shot as joe schmo with no training. It makes no logical sense to have any strike roll be a flat roll.
That said I don't play that a called shot takes an action to set up like an aimed shot does.
So a person whom is trying to shoot sombody in the head would in my game have to hit a 14 (12+2 for head being a smallish target) with more difficult to hit numbers (15 if the target is walking or 16 if the target is running or jogging & +1 for every 10 mph moving) BUT would only use one action so long as they only use their + from their WP and don't take the time to truly aim.
If they use an action to aim they get their aim bonus, and if they have the sniper skill they can use another action to add that bonus.
So in my game
Called shot = 1 action
Aimed shot = 2 actions (is automatically considered a called shot since the attacker says they "want to aim at X")
Aimed sniper shot = 3 actions (is also automatically a called shot)
Called shots in melee work the same. 12 or higher to hit and take only one action.
Decapitation or neck strikes are 16 or higher in my game and have to do the SDC of the neck and the total HP to instantly kill/decapitate. It happens alot with zombies in my DR campaign.
