When do the insanity rules come into play?
Is there a set bullet list of "use insanity rules now" or is it all up to the discretion of the GM?

Also remember something that many GM's seem to forget. Insanity allows for a saving throw and ME bonuses apply.

Rifts has some classes that pick up insanity with experience levels, like the ecto-hunter but that is not found in BtS classes. The latent can get the storms and the like but that's different obviously.
As far I recall, it's when the GM thinks the PC's brains are scrambled enough they might not go back to normal. Such as if he get possessed by some uberevil thing and survives it.

According to BTS1, p. 21 and BTS2, p. 152, "What constitutes a traumatic experience is, in part, up to the Game Master and the actual situation." Either book then provides a list of traumatic situations with guidelines about how often to check for insanity and which kind of insanity can manifest.
