Depends on the situation, how much time and resources are at the character's command.

It depends on the complexity of the situation before me.

Another factor is if I am giving orders to NPC or to PC. Orders to NPCs will be more detailed. Other PCs, who I expect to think for themselves will only get a general description of what they should do.

Give me a situation, give me some resources and I'll tell you what sort of planning I would do.

Even targets of opportunity should be attacked with a plan. The question is how much time do you have to make the plan.

Definitely. Heck you should make a plan to get out of an ambush.

The question is how much detail?

genrally depends on the situation.

in combat, i don't plan out more than 1-2 melee rounds ahead. any further than that, and your plans get junked when something happens you didn't factor in.

now, when doing infiltrations, assualts, ect, plan down to the routes taken, but have plan's B, C, D, E, ect. ready for the most likely problems.

I try to plan as far as what I think is the endgame for the current fight, but I update the plan every time circumstances require.

Yea, you need an exit strategy for anything you get yourself into. That is a big, big part of planning - the end game.

The Robotech Accelerated Training Program did similar. Basically it was a no threat of death way to get the PCs some EXP (low reward, 1/4 or less than normal) and the chance to develop teamwork and tactics.

making the players use tactics is a must IMO. If they don't use them they suffer and if I am GMing and let them get a way with un-tactical play, it is not serving the idea behind the game, again IMO.

Very true when your defending a building you can get into extreame detail, but what's the point?

Murphy steps in screws it up.

Thus our ops plans were just guidelines, even during React Drills (and Actuals) we strayed from the op plans from time to time.

But that's what the bad guys do in RL, so it's better to learn that in training then when when your clearing a room for real.
