I voted other because there are three that I would like to vote for:
- Dragon Hatchling disguised as beautiful girl
- Cyberknight as a beautiful dee bee(elf, true atlantean etc.)
- Busty Female Man at Arms
I think Ajax nailed it perfectly.

I've found it's the player, not the character that's the problem.

I'll clarify.
Players playing crazies can really bring down a game. So can any player playing any character, but insane characters seem to be more facilitating of it.

Could be, certainly. Insanity is difficult to play and not be non-disruptive. But so can mages and long bowmen, if they don't suit the player's strengths as a player.

The difference is how easily those can be used as excuses for douche-baggery.
