Is there an Extract Heart spell in Palladium? I know I've seen it for other games, but can't recall if I've seen it for Palladium.
Also, is there another Heart Attack spell besides the Blood Magic one?

The only spell I can think of is the chi magic spell, Remove Heart. This is not an attack but an enhancement as the heart becomes a gem and continues to perform its bodily functions foe the character remotely ie still pumps blood around the body.

I can't think of any 'save vs magic or insta-die' type spells in the Palladium system. Deathword is close; fail a save vs magic then you're placed in a death-like coma for 1d4 or 1d6 hours(can't recall exactly) that kills you if you fail a save vs coma when it ends.
Deathword does set the precedent, though. If you want to create a heart extraction spell, you can use the Deathword as a decent set of guidelines for level, range, PPE, etc. Add an additional save vs something on top of the normal vs magic one because insta-kill spells are a double-edged sword and annoying.

Removing the heart or causing a heart attack just seem like a normal extension of the Fleshsculptor theme. They may even be able to do this with existing spells...

How about this, using the rules in Through the Glass Darkly to invent a new spell based on Remove Tumor or Remove Living Bone. Yes, Extract Heart would be a "save or die" spell, but it would be so cool, and well within the Fleshsculptor theme!
