It's dealt with in the Bionics Sourcebook... basically, they get the 'Borg skills and most of the old ones atrophy...

I don't think they forget their skills, they just do not increase the skills in their old class when they advance a level in their new borg class.

I mean I could see people loosing abilities, casting abilities, or psionic abilities and all that jazz but why would someone forget how to do open heart surgery or how to bake a cake???

They keep their skills.
They just can't get new ones that their new class can't get.

Take a look at Doc Reid in Vampire Kingdoms.

they remember all the old skills they had. howevery they are all permanatly frozen at the level they were at and only gain new skills as a borg.

try reading the stuff they have on other converting. I belive the Main Book covered it.

The OCC change is necessary for classes that really aren't psychologically prepared for life as a cyborg such as rogue scholars, wilderness scouts, CS grunts, etc. Cyberdocs and City Rats on the other hand are extremely comfortable with cybernetic conversion and therefore adapt quickly to their new life.

Also, it's worth mentioning that "changing your OCC" to Cyborg means absolutely nothing except for experience points you need to gain. You don't change skills or anything; you merely are considered a cyborg, use the cyborg exerience table and must advance to your old level of exerience as a cyborg before you will begin to improve your old OCC's skills.

Some classes can get partial conversion and don't need to become borgs but if you go full conversion you have to switch classes.
